Dear Families,
Indiana Regional Medical Center has reached out to the District to make the COVID-19 vaccine available to any interested student age 16 or older and anyone else who might be interested. The attached paperwork will need to be completed and submitted at the time of the visit for the first dosage. The vaccine would be administered at the Kovalchick Center on Monday, May 10th (1st dose) and Tuesday, June 1st (2nd dose). IRMC will be administering the Pfizer vaccine for those interested individuals. To assist IRMC in planning, we are asking all interested families to complete a brief survey by the close of business on Friday, May 7, 2021. Click here to complete the interest survey.
To help assist interested students in receiving the vaccine, IASD will bus students to and from the Kovalchick Center. Students can sign up in the main office on Wednesday, May 5, and Thursday, May 6, for their designated times of travel. Additionally, students can call the main office on Friday, May 7, to sign up. We are estimating that the total time out of school (busing and receiving shot) will take about an hour from start to finish. A school nurse will accompany students to and from KCAC. The high school administration team will handle coordinating these efforts. Unfortunately, we can only bus students at the current time, not family members. The consent form needs to be signed by the parent/guardian.
The students will need to bring the following documentation:
- Photo ID
- Complete consent form signed by the parent/guardian
- Insurance card
I apologize for the short notice. Please understand that this is your choice as a family to decide whether or not you want to receive the vaccine. We are merely assisting those who are interested in receiving the vaccine. As always, we appreciate your support, grace, and compassion now and throughout the year.
